There's your answer then...
Do you sometimes miss your clutch timing when the car is cold, and get a bit of a bunnyhop? Even a tiny one? Cause thats fairly common... Everythings cold, less lubricated, stiffer, and the human driving is also cold, add that to a high clamping force (has slightly different loading characteristics than a lower clamp but with a higher friction material, ie, worse characteristics...)... That all add's up to a broken gearbox.
